Disma uses the collected Personal Data solely for the following business and service-related purposes:

  • Service Fulfillment: To accurately schedule, dispatch, and provide the requested Transportation Services.

  • Billing & Finance: To process invoices, handle payments, and submit claims to your insurer.

  • Communication: To send essential service notifications, updates, and information regarding your scheduled trips.

  • Operational Support: To allow authorized partners, vendors, and contractors (e.g., dispatch software providers, maintenance) to assist in the efficient delivery of our services.

During our service relationship, Disma maintains records detailing the specific transportation services you receive, including trip logs, usage patterns, and billing history. We consider this Client Service Information highly confidential.

We pledge that we will not sell or share these specific trip and usage records with unauthorized external parties. We will only disclose this information:

  • For Safety & Integrity: To investigate fraudulent activities, prevent misuse of our services, or enforce the terms of our Client Agreement.

1.1 Information Collected: We collect your telephone number(s) (including mobile numbers) when you:

  • Schedule a transportation service.

  • Sign up for service notifications or alerts.

  • Contact us via phone, web form, or mobile application.

1.2 Consent for Communication: By providing your phone number to Disma, you provide your express written consent to receive communications from us regarding your scheduled appointments, service updates, and relevant operational messages, which may be sent via text message or voice calls.

 

2. Purpose of Phone Number Use

 

Your phone number is a critical piece of information used primarily to ensure the timely and safe execution of Non-Emergency Medical Transportation (NEMT) services. We use your number for the following purposes:

  • Service Coordination: To confirm trip details, notify you of driver arrival times, or inform you of any delays or changes to your scheduled transportation.

  • Safety and Logistics: To allow our drivers or dispatchers to contact you immediately regarding your pick-up location, accessibility, or safety concerns.

  • Billing and Verification: For identity verification or to address payment-related inquiries.

 

3. Opt-Out and Communication Preferences

 

3.1 Opting Out of Text Messages: You can revoke your consent and stop receiving text message alerts at any time.

  • To STOP receiving SMS messages, reply to any text message from us with the word STOP.

  • We will send one final message confirming your successful opt-out. You will no longer receive further promotional or service-related text messages from that specific number.

3.2 Opting Out of Marketing Calls/Messages: If you wish to stop receiving marketing or promotional calls and messages, you may inform our representative during a call or contact us directly using the contact information provided in our main Privacy Statement.

3.3 Service-Critical Communication: Please note that opting out of SMS communication may prevent you from receiving vital real-time updates regarding your trip (such as driver location or delays). We reserve the right to contact you via phone or text for messages deemed necessary for the immediate completion of a service you have scheduled (i.e., service-critical operational messages).

 

4. Data Sharing and Security

 

4.1 Limited Sharing: Your phone number will be shared with the Disma personnel and technology partners (such as our dispatch and routing software provider) strictly necessary to facilitate and execute the transportation service you requested. This includes sharing the number with the designated driver assigned to your trip to coordinate pick-up.

4.2 Prohibition on Sale: We will never sell, rent, or lease your phone number or your opt-in status to any third-party marketing or telemarketing organizations.

4.3 Security: We maintain reasonable administrative, technical, and physical safeguards to protect your phone number from unauthorized access or misuse.

 

5. Fees and Charges

 

Disma does not charge any fees for you to receive text messages related to our services. However, standard message and data rates may apply to any SMS messages you send or receive, as determined by your mobile carrier plan. Please consult your wireless provider for details on your plan's rates.

This Phone Number Use & Communication Addendum is subject to change. Any updates will be reflected in the Effective Date of the main Disma Transportation LLC Privacy Statement.
